Steve: Migraine and Headache Update

Bariatric Surgery
Bariatric surgery may increase the risk for spontaneous intracranial hypotension, a study from the October issue of Neurology suggests. Cerebrospinal fluid leaks are the main cause of spontaneous intracranial hypotension, with patients presenting with a unique type of headache.

Cognitive Behavioral Therapy (CBT)
CBT is an evidence-based practice for the management of pediatric migraine. In a new study from Headache, 50% or greater headache activity reduction post-treatment and at follow-up (3 months or later) was demonstrated with CBT as compared with placebo or standard medication. Furthermore, the clinical improvement was stable, even at a 1-year follow-up. CBT is beneficial to children suffering from migraine and safe!

Device
The FDA recently approved the Cerena Transcranial Magnetic Stimulator (CTM) for people who suffer from migraines that are preceded by an aura, characterized by a tingling sensation in the extremities and flashes of light or blind spots in a person's vision. The device uses magnetic energy to stimulate the occipital cortex, an area of the brain involved in visual processing, in order to alleviate migraine pain. 

Food
According to the lead author of Diet and Headache: Part 1. Headache: The Journal of Head and Face Pain, one of the most important triggers for headache is the withdrawal of excess caffeine. One cup is 125 milligrams, probably the maximum for migraine patients.

Another trigger for migraine is monosodium glutamate (MSG), which is a flavor enhancer used in a variety of processed foods, including frozen or canned foods, soups, international foods, snack foods, salad dressing, seasoning salts, ketchup, barbecue sauce, and heavily in Chinese cooking.

Nitrites are preservatives food in processed meats such as bacon, sausage, ham and lunch meat to preserve color and flavor.

Alcohol is one of the most commonly reported dietary trigger factors for migraine and studies suggest vodka and red wines, especially those with highest histamine content, are problematic.

One of the most promising nutrients for those with more frequent attacks of migraine is one that boosts your omega-3 fats while lessoning your omega-6 levels. That means reducing corn, sunflower, safflower, canola and soy oils in favor of flaxseed oil, or better yet, pure fish/cod liver oil.

IBS: Migraine of the Bowel
Migraine and tension-type headache are linked to irritable bowel syndrome (IBS), according to a study presented at the American Academy of Neurology Annual Meeting.

The brain plays significant role in IBS. IBS patients have increased hypothalamic activity suggesting an association among IBS, stress and hypothalamic axis.

This is the first study to investigate the relationship between IBS and primary headaches. IBS occurred in 54.2% of patients with migraine and 28.3% of patients with tension-type headache.

Analysis also showed the serotonin transporter gene in those with IBS, migraine, and tension-headache groups had at least one gene that differed from the genes of the healthy participants.

Melatonin
Melatonin may be an option for migraine sufferers, according to the study in the May issue of Journal of Neurology and Neurosurgery in Psychiatry. In those taking 3 mg. melatonin, migraine was 19.4% less than placebo, migraine intensity was 28.1% less than placebo, and average migraine duration was 39.6% less than placebo.

Menstruation
Many women who have migraine toward the end of menstruation may be best treated with iron supplements, a new study from Headache suggests.

A new type of headache dubbed "end-menstrual migraine" found that many women with this condition, most of whom were found to have low levels of ferritin, used iron supplementation as a suitable treatment.

The researchers believe that end-menstrual migraine is not hormonally mediated but rather causally related to menstrual blood loss, resulting in a brief relative anemia with consequent migraine.

In a recent study from Headache, results highlighted the potential impact of excessive internet and mobile use, which ranges from different types of headache to other somatic symptoms in adolescents.

Do You Convert or Stoke the Fire?

Bonnie and Steve: Are you aware that at least one quarter of the human population has difficulty converting fats from food into antiinflammatory omega-3 fatty acids EPA and DHA? 

Because of a genetic mutation of the FADS1 gene, many of us have to take the supplements, such as fish oil or krill oil that go into our bodies directly as EPA and DHA, and do not have to be broken down.

Flaxseed, canola oil, and just about any oil in your diet has to go through several enzymatic and genetic processes to end up as EPA and DHA.

There are two ways you can circumvent this issue.
  1. Take fish or cod liver oil to supplement your diet as an insurance policy, especially if you have, or have in your family history, an inflammatory-related disorder.
  2. Test to see if you have a mutation of the FADS1 gene. The severity can be 50% mutation (heterozygous) or 100% mutation (homozygous).
Knowing if you are a good converter is fairly important, as the following research confirms.

In a study reported in the Nutritional Neuroscience, researchers found that people with higher blood levels of omega-3's tended to have larger left frontoparietal brain cortices, and the size of the frontoparietal cortex predicted better performance on tests of fluid intelligence.

While the research tells us that people need to be eating fish and fish oil to get neuroprotective effects, the new finding suggests that we can get a similar benefit from fats from nuts, seeds and oils, but only if we can transfer them into EPA and DHA.

In a second study from Aging and Disease, the same research team found that the size of the brain fornix was associated with memory preservation in older adults.

Both of these findings have important implications for poor converters, because the Western diet tends to be largely imbalanced with high amounts of omega-6 fatty acids and low amounts of omega-3 fatty acids.

If we cannot convert those genetically, we are stoking the inflammatory fire!

Gastric Banding Reoperation Common
Steve: A new study in May's JAMA has called into question the safety and effectiveness of gastric banding.

Over the course of nearly 5 years of follow-up, a study found that almost one in five Medicare beneficiaries who had undergone gastric band placement required reoperation.

The study was the first long-term national analysis of gastric-band related reoperations in the United States.

Taken together, these findings indicate that the gastric band is associated with high reoperation rates and considerable costs to payers, which raises concerns about its safety, effectiveness, and value.

Learn How to Be a Smarter Shopper
Bonnie: There is a battle in the supermarket isle, a battle between what the consumer wants and what the retailer and others want the consumer to see, and subsequently to buy.

Product packages and displays contain a number of features and attributes tailored to catch consumers' attention. These are called external factors comprising the visual saliency (how items stand out), the number of facings, and the placement of each product.

Researchers in a September Appetite study fit mobile eye trackers to consumers visiting real-life supermarkets in order to investigate to what extent external and internal factors affect consumers' visual attention and purchases.

Consumers appear to take advantage of visual saliency in their decision making, using their knowledge about products' appearance to guide their visual attention towards those that fit their preferences.

When it comes to actual purchases, however, visual attention was by far the most important predictor. In other words, the very act of looking longer or repeatedly at a package, for any reason, makes it more likely that this product will be bought.

Visual attention is thus crucial for understanding your consumer behavior. Just because you look at a product longer does not mean you have to buy it!

How Much Gluten Is In Gluten-Free Food?
Steve: More than 99.5 percent of sampled food products labeled "gluten-free" are in compliance with FDA's requirement that such foods have less than 20 ppm (parts per million) of gluten.

This is the finding of an FDA sampling assignment in which 702 samples from more than 250 products labeled "gluten free" were collected and analyzed. Only one of those products was found to not comply with one of the major FDA requirements that went into effect in August 2014 for manufacturers using the term "gluten-free" on their labeling.

Long-Term Stress and the Effect on the Fetus
Bonnie: The feeling of constantly being on edge, always having to take care of everything, not being able to find a balance: If an expectant mother is strongly stressed over a longer period of time, the risk of the unborn child developing a mental or physical illness later in life, such as attention deficit hyperactivity disorder (ADHD) or cardiovascular disease, increases.

The precise mechanism of how stress affects the baby in the womb is not yet been completely clarified, physical stress to the mother can change the metabolism in the placenta and influence the growth of the unborn child.

When stressed, the human body releases hormones to handle the higher stress, such as the so-called corticotropin-releasing hormone (CRH), which results in an increase in stress hormone cortisol. This mechanism also persists during pregnancy, and the placenta, which supplies the fetus with nutrients, can also emit stress hormone CRH. As a result, a small amount of this hormone enters the amniotic fluid and fetal metabolism.

Short-term stress situations, however, do not seem to have an unfavorable effect on the development of the fetus.

The psychologists, writing in the May issue of Stress, advise pregnant women who are exposed to longer-term stress situations to "seek support from a therapist to handle the stress better." Stress during pregnancy cannot always be avoided, however. "A secure bond between the mother and child after the birth can neutralize negative effects of stress during pregnancy," the authors says.

Wristband and Tracking Apps Have Major Question Marks
Steve: The wristband tracking devices worn by millions of fitness-conscious Americans do an adequate job of measuring heart rate, but not of estimating energy expenditure, reports a study in the May issue of Journal of Personalized Medicine.

Even the most accurate device was off by an average of 27.4%, and the least accurate erred by 92.6%.

The results suggest the need for caution in using energy outlay readings from these units to guide health improvement and weight management programs.

In addition, only half a percent of applications regarding weight management available for mobile devices have been developed with the help of professional healthcare input.

In a study of almost 29,000 apps across five mobile app stores in 10 countries, presented at the European Obesity Summit, only 17 were verifiably based on professional expertise.

Bonnie: Bone/Collagen-Building Soup
-4 oz. organic chicken breast, cut into cubes
-2 T. extra virgin olive oil
-8 oz. Pacific Foods organic Bone Broth (9 gm. Protein and collagen) OR Bonafide Drinkable Veggie Bone Broth
-6 oz. low sodium V-8 juice
-1/2 cup dry white wine/water/veggie broth (or a combination)
-1/2 medium size yellow onion, finely diced
-one medium size shallot, finely diced
-1/2 cup organic carrots, diced
-1/4 cup organic celery, diced
-1/4 cup organic red pepper, diced (optional)
- 1/2 cup organic curly parsley (whole pieces)
-iodized sea salt and pepper to taste
-1/2 of a 15 oz. can organic white or black beans, thoroughly rinsed

Saute all diced vegetables in olive oil until the onion is translucent. Add the cubed chicken and sauté until the chicken is no longer pink. Add a small amount of liquid, if the chicken begins sticking. When the chicken is cooked through, add all other liquids and parsley. Bring to a boil, then simmer, partially covered, for 30-45 minutes. Add the beans and cook for 15 minutes longer. Remove parsley and serve with a garnish of Parmesan/Romano cheese or freshly diced parsley. SERVES: 2

Update on Achilles Tendon Rupture Treatment
Steve: Although goals of treatment have not changed in the past 15 years, a recent study in the May issue of Journal of the American Academy of Orthopaedic Surgeons, suggest nonsurgical choice is viable.

Of nonsurgical management, specifically functional bracing with early range of motion, demonstrate rerupture rates similar to those of tendon repair by surgery and result in fewer wound and soft-tissue complications.

There is no improvement in functional outcomes or tendon properties with the use of platelet-rich plasma, but promising results with the use of bone marrow-derived stem cells have been seen in animal models.
