DID YOU KNOW that despite having guidelines in place to manage conflicts of interest, the vast majority of medical organizations that produce clinical guidelines do not disclose the organizations' own ties with biomedical companies in their published guidelines, according to a study in last week's PLOS Medicine. The Institute of Medicine Committee on Conflict of Interest in Medical Research, Education, and Practice recommends that organizations disclosure all direct and indirect funding sources with each guideline.

What We Know About Reflux Is Wrong
Bonnie and Steve: New research could destroy the long-held belief that reflux is caused by acid burning.

For more than 80 years, it has been assumed that stomach acid backing up through the esophagus damaged the lining of the esophagus by causing chemical burns, but research in last month's JAMAsuggests that much of the damage in patients with gastroesophageal reflux disease (GERD) actually occurs through an inflammatory response prompted by the secretion of proteins called cytokines.

The lead researcher was quoted saying, "someday we might treat GERD with medications that target the cytokines or inflammatory cells that really cause the damage to the esophagus". Wow. Sobering for those who have dealt with the side effects from reflux medications.

While it is believed that it takes several weeks from the time stomach acid is introduced into the esophagus before damage occurs, in reality, a chemical burn should develop immediately, as it does if you spill battery acid on your hand. This is why researchers thought that something else was also at play.

In the study, the researchers looked at patients who had reflux esophagitis (injury to the esophagus) that had been successfully treated by proton pump inhibitors (PPIs). The researchers thought that GERD might redevelop if PPIs were stopped, providing an opportunity to observe the early changes of GERD.

Changes to the esophagus reoccurred after the PPIs were stopped. Importantly, the changes that re-occurred were not consistent with chemical burns. Rather, the findings supported the new idea that refluxed stomach acid stimulates the esophagus to make small proteins called cytokines, which then sets up the process of inflammation.

So what does it all mean? Blocking stomach acid with medications is not the long-term solution. Short-term medication use while reducing the inflammation is a better solution.

Why does one individual who produces the same stomach acid as another individual get reflux and the other does not? Because one individual is more prone to inflammation. Doctors need to be made aware of this immediately.

Bonnie and Steve: Much to our chagrin, UAS Life Sciences has discontinued UP4 Acidophilus (yellow label). Their reasoning was: "it's not a big seller" and "customers want multiple strains per capsule".

When we mentioned that in many instances, those who suffer from yeast imbalances need and can only tolerate acidophilus, they had no credible response. The cold reality is that even supplement manufacturers go with what sells, not what serves the best interest of its diverse customers.

Luckily, we have options to cater to your individual needs.
  1. If you no longer struggle with yeast imbalance, you can switch to UP4 Daily (lactobacillus acidophilus and bifidus).
  2. For everyone else, we contract manufactured our own Acidophilus product: Well Probiotic L. Acidophilus. We will ship this product with refrigeration during the warmer months. but you can store it exactly as you did with UP4 Acidophilus (just refrigerate after opening).
We have never been about "what sells". Unfortunately, we are penalized for this as evidenced by the discontinuation of certain Twinlab products and now this.
