Options
In order to come up with the most optimal eating and lifestyle plan, we must assess your daily eating habits, medical history, family and genetic history, health-related symptoms, and lifestyle choices.
Please pay attention to our checklist of what you need for your visit, including questionnaire, supplements, medications, bloodwork, and genetic data (optional).
*We work with Long COVID.
*We screen for Food Reactions and Gut Dysbiosis.
We encourage in-office visits if you live in the Chicagoland area. Bonnie and Steve do phone consultations. Steve also does telehealth.

ARE WE COVERED BY INSURANCE?
Bonnie's services may be covered when prescribed by a physician for "medical nutrition therapy" with a diagnosis code(s). Upon request, we will give you a superbill receipt to submit with your Rx and claim form.
Steve's services may be covered with Health and Well-Being CPT Codes and a doctor's prescription. Please ask us to add this to your receipt if you are going to submit to insurance.

Most FSA and HSA health spending accounts cover our services.
Payment is expected upon receipt of services. We do not submit to insurance for you.

FOOD REACTION SCREENING
If you would like to get our food reaction screening as part of your initial visit, we suggest emailing Bonnie or Steve first to explain why you think you need it.
